Welcome to iv-revive.com (the "Site"), operated by CVManagement Inc. doing business as IVREVIVE ("IVREVIVE," "we," "us," or "our"). These Terms of Use ("Terms") govern your access to and use of the Site. By accessing or using the Site, you agree to be bound by these Terms and our Privacy Policy, which is incorporated here by reference. If you do not agree, please do not use the Site.
1. About IVREVIVE
IVREVIVE is a wellness studio located at 17 Lumber Road, Roslyn, New York. The Site is owned and operated by CVManagement Inc. Clinical and medical services described on the Site are provided by licensed healthcare professionals affiliated with IVREVIVE. CVManagement Inc. provides administrative, marketing, and management services in support of the practice but does not itself provide medical care.
2. Acceptance of Terms
By using the Site, you represent that you are at least 18 years of age and have the legal capacity to enter into a binding agreement. If you are using the Site on behalf of another person or entity, you represent that you have the authority to bind that person or entity to these Terms.
3. No Medical Advice; No Provider-Patient Relationship
The information contained on the Site, including descriptions of services, treatments, ingredients, and wellness benefits, is provided for general informational and educational purposes only. It is not intended to be, and should not be construed as, medical advice, diagnosis, or treatment.
Use of the Site does not create a physician-patient, nurse-patient, or any other provider-patient relationship between you and IVREVIVE or any of its affiliated healthcare professionals. A provider-patient relationship is established only after you book an appointment, complete the required intake and consent forms, and are evaluated in person by a licensed clinician at our studio.
You should always seek the advice of a qualified healthcare professional regarding any medical condition or before starting any new treatment, supplement, or wellness regimen. Never disregard professional medical advice or delay seeking it because of something you have read on the Site.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
4. Booking and Third-Party Platform
Appointments with IVREVIVE are scheduled through a third-party booking platform operated by Nursepreneurs. When you book an appointment, you will be directed to the Nursepreneurs platform, where you will be asked to provide health information, review intake forms, and acknowledge a Notice of Privacy Practices under the Health Insurance Portability and Accountability Act (HIPAA).
Your use of the Nursepreneurs platform is governed by Nursepreneurs' own terms of service and privacy policy, which are separate from these Terms. IVREVIVE is not responsible for the practices, content, or availability of the Nursepreneurs platform, and your interactions with that platform are at your own risk.
5. Lead Capture and Communications
The Site allows you to submit your name, email address, phone number, and other contact information through forms, newsletter sign-ups, and similar features. By submitting your information, you consent to be contacted by IVREVIVE regarding our services, promotions, events, and related communications.
5.1 Email Communications
By providing your email address, you consent to receive emails from IVREVIVE, including marketing and promotional emails. You may unsubscribe at any time by clicking the unsubscribe link in any email or by contacting us at info@iv-revive.com.
5.2 SMS and Text Message Communications
By providing your mobile phone number, you expressly consent to receive text messages from IVREVIVE, including appointment reminders, service updates, and marketing or promotional messages, at the number you provide. Consent is not a condition of purchasing any goods or services.

6. Permitted Use of the Site

Impersonate or attempt to impersonate IVREVIVE, an IVREVIVE employee or affiliated provider, another user, or any other person;
Use any automated means, including bots, scrapers, or data-mining tools, to access or collect information from the Site;
Attempt to gain unauthorized access to, interfere with, damage, or disrupt any portion of the Site or any server, computer, or database connected to the Site;
Submit false, misleading, or fraudulent information through any form on the Site.
7. Intellectual Property
All content on the Site, including text, graphics, logos, images, photographs, videos, icons, designs, and the IVREVIVE name and trademarks, is the property of CVManagement Inc. or its licensors and is protected by United States and international copyright, trademark, and other intellectual property laws.
You may view, download, and print content from the Site solely for your personal, non-commercial use, provided that you do not modify the content and that you retain all copyright and other proprietary notices. Any other use, including reproduction, modification, distribution, transmission, republication, display, or performance of the content, is strictly prohibited without our prior written consent.

The Site may contain links to third-party websites, services, or platforms, including the Nursepreneurs booking platform and our social media profiles. These links are provided for your convenience only. IVREVIVE does not control and is not responsible for the content, accuracy, privacy practices, or availability of any third-party websites or services. Your access to and use of any third-party site or service is at your own risk and subject to that third party's terms and policies.
9. Disclaimers
THE SITE AND ALL CONTENT, INFORMATION, AND MATERIALS PROVIDED ON OR THROUGH THE SITE ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED. TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, IVREVIVE DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, AND ANY WARRANTIES ARISING OUT OF COURSE OF DEALING OR USAGE OF TRADE.
IVRevive does not warrant that the Site will be uninterrupted, secure, or free of errors, viruses, or other harmful components, or that any defects will be corrected. IVRevive makes no representations or warranties regarding the accuracy, reliability, completeness, or timeliness of any information on the Site.
Statements about the potential benefits of any wellness service, treatment, or product described on the Site have not been evaluated by the U.S. Food and Drug Administration. Services and treatments are not intended to diagnose, treat, cure, or prevent any disease. Individual results may vary.
10. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, IN NO EVENT SHALL CVMANAGEMENT INC., IVREVIVE, OR ANY OF THEIR OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, AFFILIATED HEALTHCARE PROFESSIONALS, OR LICENSORS BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, PUNITIVE, OR EXEMPLARY DAMAGES, INCLUDING DAMAGES FOR LOSS OF PROFITS, GOODWILL, USE, DATA, OR OTHER INTANGIBLE LOSSES, ARISING OUT OF OR RELATING TO YOUR ACCESS TO OR USE OF, OR INABILITY TO ACCESS OR USE, THE SITE OR ANY CONTENT ON THE SITE.

11. Indemnification
You agree to indemnify, defend, and hold harmless CVManagement Inc., IVREVIVE, and their respective officers, directors, employees, agents, affiliated healthcare professionals, and licensors from and against any claims, liabilities, damages, judgments, awards, losses, costs, expenses, or fees (including reasonable attorneys' fees) arising out of or relating to your violation of these Terms or your use of the Site, including any use of the Site's content other than as expressly authorized in these Terms.
12. Governing Law and Venue
These Terms and any dispute arising out of or relating to these Terms or the Site shall be governed by and construed in accordance with the laws of the State of New York, without regard to its conflict of laws principles. You agree that any legal action or proceeding arising out of or relating to these Terms or the Site shall be brought exclusively in the state or federal courts located in Nassau County, New York, and you consent to the personal jurisdiction of such courts.
13. Changes to These Terms
We may update these Terms from time to time. When we do, we will revise the "Last Updated" date at the top of this page. Your continued use of the Site after any changes constitutes your acceptance of the revised Terms. We encourage you to review these Terms periodically.
14. Severability
If any provision of these Terms is found by a court of competent jurisdiction to be invalid, illegal, or unenforceable, that provision shall be enforced to the maximum extent permissible, and the remaining provisions shall remain in full force and effect.
15. Entire Agreement
These Terms, together with our Privacy Policy, constitute the entire agreement between you and CVManagement Inc. regarding your use of the Site and supersede any prior agreements or understandings, whether written or oral, relating to the same subject matter.
