This Privacy Policy explains what information we collect when you visit iv-revive.com (the "Site") or interact with our wellness studio at 17 Lumber Road, Roslyn, NY, how we use it, who we share it with, and the choices you have.

Information We Collect

We collect information in a few different ways depending on how you interact with us.

Information you provide directly. When you fill out a contact form, sign up for our email list, request an appointment, or complete an intake form, you may provide your name, email address, phone number, mailing address, date of birth, emergency contact information, and details about your health history, current medications, allergies, symptoms, wellness goals, and other clinical information relevant to the services you are seeking.

Information collected automatically. When you visit the Site, we and our third-party providers automatically collect certain technical information, including your IP address, device type, browser type, operating system, referring URL, pages viewed, time spent on the Site, and similar usage data. This is collected through cookies, pixels, and similar tracking technologies described below.

Information from third parties. When you book an appointment through Nursepreneurs, our scheduling and patient management provider, Nursepreneurs may share booking and account information with us. If you interact with our content on social media platforms or click through one of our ads, those platforms may share limited information with us.

How We Use Your Information

We use the information we collect to:

  • Provide, schedule, and deliver the wellness services you request

  • Maintain accurate clinical and treatment records

  • Communicate with you about appointments, follow-ups, and care

  • Send you marketing emails, newsletters, and promotional offers (only if you have opted in)

  • Respond to inquiries submitted through our contact form

  • Operate, improve, and secure the Site

  • Measure the performance of our marketing and advertising

  • Comply with legal, regulatory, and professional obligations

Protected Health Information and HIPAA

Health information you provide through intake forms, consultations, and the course of treatment is considered Protected Health Information (PHI) and is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable New York State health privacy laws. Our handling of PHI is governed by our Notice of Privacy Practices, which is provided to you at your first visit and available upon request. In the event of any conflict between this Privacy Policy and our Notice of Privacy Practices with respect to PHI, the Notice of Privacy Practices controls.

Cookies, Analytics, and Advertising Technologies

The Site is built on the Squarespace platform, which sets cookies necessary to operate the Site and to provide basic analytics about Site usage. We also use the Meta Pixel and Google advertising and analytics technologies to understand how visitors use the Site, measure the performance of our advertising, and deliver more relevant ads on Facebook, Instagram, Google, YouTube, and partner sites.

Some browsers offer a "Do Not Track" signal. The Site does not currently respond to these signals.

Email Marketing

If you opt in to receive emails from IVREVIVE, we send communications through Squarespace Email Campaigns and Mailchimp, which process your name, email address, and engagement data (opens, clicks) on our behalf. Each platform maintains its own privacy practices, which you can review at squarespace.com/privacy and mailchimp.com/leg/privacy.

You can unsubscribe at any time using the link at the bottom of any marketing email, or by contacting us directly. Unsubscribing from marketing emails does not stop transactional communications related to your appointments or care.

How We Share Your Information

We do not sell your personal information. We share information only as needed to run the studio and comply with the law, including with:

  • Service providers who help us operate the Site and run the business, including Squarespace (website hosting and email marketing), Nursepreneurs (appointment scheduling and patient management), Mailchimp (email marketing), and Meta and Google (advertising and analytics). These providers are contractually required to protect your information and use it only for the purposes we authorize.

  • Healthcare professionals involved in your care, consistent with HIPAA and our Notice of Privacy Practices.

  • Legal and regulatory authorities when required by law, subpoena, court order, or to protect the rights, safety, or property of IVREVIVE, our patients, or others.

  • Successors in the event of a merger, acquisition, financing, or sale of all or part of our business.

Data Security

We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit, access controls, and secure storage of clinical records. No system is perfectly secure, however, and we cannot guarantee absolute security. If we learn of a breach affecting your personal information, we will notify you as required by applicable law, including the New York SHIELD Act.

Your Choices and Rights

Depending on where you live, you may have rights regarding your personal information, including the right to access, correct, or delete information we hold about you, and the right to opt out of marketing communications. New York residents have specific protections under state law, and patients have additional rights under HIPAA with respect to PHI, including the right to request copies of and amendments to their medical records.

Children's Privacy

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 without verifiable parental consent. If you believe a child has provided us with personal information, please contact us and we will take appropriate steps to delete it.
